Cleanroom Connection, a supplier of cleanroom and compounding products to pharmacy, healthcare, and pharmaceutical facilities since 2003, today released a free USP 800 PPE checklist. It is built for compounding pharmacies, hospital pharmacies, and oncology preparation areas that handle hazardous drugs.

PPE failures in hazardous drug environments are rarely about the product. They happen when staff wear the right glove or gown incorrectly, change them too late, or skip them during tasks that feel lower risk but still carry exposure.

Most compliance reviews confirm that a facility owns the correct PPE and that staff completed training. Fewer check what happens on the floor. The checklist is designed for that second review. It helps pharmacy directors and compliance leads compare written SOPs against actual practice across every shift.

Common gaps the checklist is meant to surface include:

  • Outer gloves not changed often enough during long compounding sessions
  • Gowns worn past their intended use period during busy stretches
  • Eye and face protection skipped on tasks that feel routine
  • Different gowning and glove habits between day and evening shifts
  • Training records that show completion but no observed competency

The checklist is organized into four review areas:

  • Gloves: chemotherapy rating and permeation testing, double gloving where required, outer glove change intervals, and changes at workflow transition points.
  • Gowns: disposable low-permeability material, full arm and torso coverage with secure closures, change intervals, and consistency across shifts.
  • Additional PPE: eye and face protection for splash risk, masks versus respirators by task, sleeve and shoe covers, and whether PPE is stocked at the point of use.
  • PPE use and workflow: donning and doffing sequence, change points before and after hazardous drug handling, observation-based reinforcement, and hazardous waste disposal of used PPE.

Facilities can use the checklist for internal audits, PPE and supplier evaluations, and staff training refreshers. It does not replace a facility’s own policies or regulatory requirements.

The USP 800 PPE checklist is available free at Cleanroom Connection’s website.
